THE PRICE RISES WITH DOSE, AND THE PRODUCT PAGES DO NOT SAY SO. The four product pages print a single flat figure each ($299/mo semaglutide injection, $299/mo semaglutide tablets, $399/mo tirzepatide injection, $399/mo tirzepatide tablets) and the homepage FAQ adds "Pricing is transparent, with no hidden fees." Read directly off the profile page, not a search snippet. Claimed profile on a paid Trustpilot subscription, replies to 100% of negative reviews. The 3.9 is bimodal rather than middling: 57% five-star, 32% ONE-star, 11% in between. ⚠️ Willow's own product pages display "4.9/5 (2200 + 5 star reviews)". WHAT IT GETS RIGHT, VERIFIED: the legal entity is named with a street address (Willow Health Services, Inc., 8080 Westpark Drive STE 42292, Houston, TX 77063, Texas governing law); all five partner pharmacies are named in the Terms of Service with full addresses and phone numbers (Red Rock Home Pharmacy, Springville UT; Epiq Pharmacy / Epiq Scripts LLC, Richardson TX; Absolute Pharmacy, Lutz FL; The Pharmacy Hub, Miami FL; Empower Pharmacy, Houston TX); 36 physicians are listed by name with MD/DO credentials and individual bio pages including medical school and residencies, headed by a named CMO (Dr. Michael Green, MD, board-certified in Family Medicine and OB/GYN, 30 years); the compounded-drug disclaimer is present verbatim and prominently on every page; and cancellation and refund policies are both published rather than merely asserted. (1) THE 4.9 RATING ON THE PRODUCT PAGES IS NOT WHAT ITS TRUSTPILOT PROFILE SAYS. Willow's product pages display "4.9/5 (2200 + 5 star reviews)". Opened directly on 2026-09-16, trustpilot.com/review/startwillow.com, a profile Willow claimed in December 2024 and pays for a Trustpilot subscription on, reads 3.9 out of 5 across 354 reviews, 114 of them in the last 12 months. The distribution is split rather than merely average: 57% five-star and 32% one-star, with only 11% in between. Willow does not attribute its 4.9 to Trustpilot, so this is not the HumeCare case of displaying a platform's branded score after that platform pulled the record. The profile is live, claimed and rated. But the volume claim is off by more than six times and the rating by a full point against the one independent corpus that exists for this clinic, and Willow replies to 100% of its negative reviews, so it reads them. The escalation is published (in a help-center article on a different host (docs.startwillow.com) and in Terms of Service §5, which promises to disclose the new amount and obtain consent before charging) but nowhere on the product pages, which print one flat number each under the homepage's claim that "Pricing is transparent, with no hidden fees." Since doses are titrated upward as a matter of normal protocol, treat the headline as a starting point. (3) THE STATES LIST ON /about DID NOT MATCH ITS OWN CHECKOUT. Willow's footer links to a LegitScript keyword SEARCH for startwillow.com rather than to a certificate, and LegitScript's checker returned 403 to direct lookup and requires a reCAPTCHA in-browser (2026-09-16). This is the facility FDA inspected. Issued by Center for Drug Evaluation and Research (CDER). Subject: CGMP/Finished Pharmaceuticals/Adulterated/Unapproved New Drug. Posted 2026-09-22. No closeout letter posted.
Following an inspection of the Houston facility at 7601 N. Sam Houston Pkwy W., Ste. 100 from November 3 to November 14, 2025, FDA states that the pharmacy "compounded drug products that are essentially copies of commercially available drug products regularly and in inordinate amounts", naming TIRZEPATIDE/NIACINAMIDE and SEMAGLUTIDE/CYANOCOBALAMIN injections, so those products lost the section 503A exemptions and are described as unapproved new drugs and misbranded. FDA also states that sterile products were "prepared, packed, or held under insanitary conditions, whereby they may have become contaminated with filth or rendered injurious to health" (inadequate smoke studies in the ISO 5 area, media fills not run under the most challenging conditions) and lists five CGMP violations. FDA found some corrective actions adequate, could not evaluate others, and called the documentation of the January 2026 media fill deficient.

Following an inspection from September 24 to October 4, 2024, FDA states that drug products intended or expected to be sterile were "prepared, packed, or held under insanitary conditions, whereby they may have become contaminated with filth or rendered injurious to health," making them adulterated under FDCA 501(a)(2)(A). Three specific observations: routine environmental monitoring did not sample within the critical area where aseptic processing occurs; media fills were not performed under the most challenging or stressful conditions, so there is "a lack of assurance that your firm can aseptically produce drug products"; and the stopper sorting bowl, supply hopper and insertion station contacting stoppers used for injectable products were not sterilized. FDA found some corrective actions adequate but could not fully evaluate others for lack of supporting documentation.
