LttlReviews & Cost (2026)
By Alexandru Vieru · Founder & Editor · Not medical advice
Independently researched · scored on our public 4-criteria methodology
Compounded semaglutide from $89 a month on a 12-month plan billed $1,068 up front, or $99 month to month at every dose, with tirzepatide from $139. It names four partner pharmacies with addresses, but its own pages advertise 125,000 members and a 4.5-star rating that its 12-review Trustpilot profile does not support.
Short answer
Lttl scores 7.2/10 (#27 of 61 clinics). $89/mo on the 12-month plan, billed $1,068 annually is the lowest tier Lttl lists, but the page doesn't spell out the commitment behind it. Treat it as a floor, not a quote for what you'd actually pay. There is also a flagged concern. See Is Lttl legit? below.

Pricing details
$89/mo on the 12-month plan, billed $1,068 annually is the lowest tier Lttl lists, but the page doesn't spell out the commitment behind it. Treat it as a floor, not a quote for what you'd actually pay.
Scored on $89/mo for compounded semaglutide on the 12-month supply, billed $1,068 annually (read 27 October 2026 on lttl.com/plans). The same drug is $93/mo on 6 months ($558), $96/mo on 3 months ($288) and $99/mo month to month, billed $99 every month with no higher rate after the first month. Compounded tirzepatide is $139/$143/$146/$149 on the same four plan lengths ($1,668 a year). The rate is the same at every dose. Microdose plans are separate and cost more: $109 to $129 a month on 3 to 12 months, or $119 for the first month and then $149 a month. A struck-through "regular" price ($249 for semaglutide) sits beside every rate; nobody is shown paying it.
What you actually pay
- First payment
- The whole plan term at checkout: $1,068 for 12 months of compounded semaglutide ($89 a month), $558 for 6 months, $288 for 3 months, or $99 for one month. Tirzepatide: $1,668, $858, $438 or $149. Lttl says you are charged only after a provider reviews your assessment.
- Recurring cost
- The same rate at every dose. Month to month semaglutide stays $99 a month; the plan page shows "Billed as: $99/mo every month". Plans renew automatically until you cancel.
- Commitment
- None on the monthly plan. The 3, 6 and 12-month plans are paid up front, and the checkout terms say multi-month packages are not refunded for unused medication or remaining months.
- Cancelling
- Email hello@lttl.com; cancelling stops future shipments and charges, and an order already processed by the pharmacy cannot be cancelled or refunded. Refunds are limited to a provider turning you down at the first consultation or a billing error, requested in writing within 28 days.
- HSA/FSA and insurance
- Lttl does not bill insurance. It says its plans are HSA and FSA eligible and gives an itemized receipt.

Questions to ask Lttl at intake
Each one is here because Lttl’s pages left it open when we read them.
- Do I need lab work before starting, and what does it cost? Its pages don't say whether labs are needed or what they cost.
- What will I pay each month after the introductory price ends? Lttl advertises a first-period price that differs from its ongoing rate.

Independently verified on Trustpilot: 4.2/5 from 12 reviews (checked 2026-10-27). 12 of those (100%) are from the last 12 months. A rating resting mostly on old reviews reads differently than one built on recent ones. Domain check passed: the profile's contact info lists lttl.com, hello@lttl.com and (888) 847-7806, identical to the site footer. Read directly in Chrome on 2026-10-27 (HTTP 403 status, full profile rendered). Claimed profile on a paid Trustpilot subscription, invites reviews, replied to 100% of negative reviews. Watch out for
(1) THE RATING AND MEMBER COUNTS ON ITS PAGES ARE NOT THE RECORD WE CAN SEE. Lttl's pages show "4.5★ from 125,000+ Members", "Excellent TrustPilot 125,000+ happy customers", "Join 500,000+ others" and "Trusted by 100k+ Americans". Its claimed Trustpilot profile (trustpilot.com/review/lttl.com, read 27 October 2026) holds 12 reviews at 4.2. The site also shows "Forbes Health's Best Provider of 2026" and a "LegitScript Certified" badge; we could not confirm either (LegitScript's checker sits behind a captcha), so we claim neither. (2) OTHER PAGES THAT DISAGREE. Product pages say "Arrives in 1-2 days"; the shipping policy says 2 to 5 business days at the pharmacy, then 3 to 7 business days in transit. A site-wide banner offers "Up to $140 off your first month", but the semaglutide and tirzepatide injection plans carry no first-month discount. The intake says you can cancel "without speaking to anyone"; the FAQ and refund policy say to email hello@lttl.com. (3) PREPAID PLANS ARE FINAL. The 3, 6 and 12-month plans are charged up front, and the checkout terms say "multi-month packages will not be refunded for unused medication or remaining months" and that "cancellation fees may apply", with no amount given. Refunds are limited to being turned down at the first consultation or a billing error, asked for in writing within 28 days. (4) WHO PRESCRIBES IS NOT NAMED. Prescribing is by "independent networks of U.S.-licensed clinicians". The medical advisory board it names is a pharmacist, a family nurse practitioner and a physical therapist, and it says the board reviews protocols, not patients. (5) LOUISIANA. The intake offers Louisiana residents only brand-name Ozempic or Wegovy, with no published price, so the compounded plans scored here are not available there. WHAT WE CHECKED: FDA's warning-letter database has no rows for "Lttl", for its operator The Commerce Group Holdings, LLC, or for any of its four named pharmacies (27 October 2026). How we know. The evidence behind this score
Every criterion below shows what we checked, when, and where you can check it yourself. Where we have no public source on file, the row says so rather than implying one exists. 4 of 4 criteria here are backed by a source you can open.
How Lttl works
- 1. Online intake. Complete a medical questionnaire and eligibility check (often BMI-based) covering your health history.
- 2. Clinician review. A licensed clinician reviews your information and, if appropriate, prescribes semaglutide or tirzepatide with a dose-titration plan.
- 3. Delivery & monitoring. Your prescription is filled by a pharmacy and sent to you, and your clinician follows up on dose changes along the way.
