Pricing details

$99/mo on a 12-month plan, $1,188 paid upfront is a real, ongoing rate on the plan it applies to. Our evidence shows no sign it's a promotional rate that rises later.

FormBlends publishes every plan with the amount charged. Compounded semaglutide is $1,188 for 12 months ($99 a month), $714 for 6 months or $399 for 3 months. Month to month it is $190 after a $99 first month; that first-month price does not apply to the 12-month plan, so the $99 a month on it is not an introductory rate. Compounded tirzepatide is $149 for the first month, then $239 a month, or $2,148 for 12 months ($179 a month). Each plan is paid once, up front, and covers medication, supplies, the provider review and shipping. Lab work is extra. Read 2026-10-14.

Questions to ask FormBlends at intake

Each one is here because FormBlends’s pages left it open when we read them.

  1. How do I cancel, and is anything refunded if I stop partway through a plan? We didn't find FormBlends's cancellation terms on the pages we read.
  2. What will I pay each month after the introductory price ends? FormBlends advertises a first-period price that differs from its ongoing rate.
  3. Is the longer plan charged up front, and can I switch to month-to-month later? At least one of its plans is billed for several months at once.
  4. Does my price change when my dose goes up? Its plan prices don't say whether they hold at higher doses.
  5. Can I use insurance or an HSA/FSA card for any part of this? We didn't find a statement from FormBlends about insurance.
  6. How do I reach a clinician between visits, and how fast do they reply? We didn't find a published support or messaging commitment.

Watch out for

FormBlends prints a full price list and its billing rules; what does not hold up is the trust signal on its sign-up page. (1) A TRUSTPILOT RATING WE COULD NOT FIND. Its GLP-1 sign-up page shows "Excellent 4.8 ★★★★★ Trustpilot · 12,400+ happy customers". Trustpilot has no page for formblends.com ("The page you're looking for could not be found"), and a Trustpilot search for FormBlends returns no results (checked 14 October 2026). BBB has no profile either, so there is no independent record behind the rating. (2) THE $99 FIRST MONTH IS INTRODUCTORY. Month to month, semaglutide is $99 for the first month and $190 after; tirzepatide is $149 and then $239. The $99 a month that lasts is the 12-month plan, $1,188 paid at once. (3) PAID UP FRONT. Every plan is a one-time charge for its full length, and dispensed medication cannot be returned or refunded. The sign-up page says "Cancel Anytime"; in the refund policy that means stopping the next charge, not getting back a plan already paid. The same page calls the medications "Safe & Effective", while stating that the compounded drugs are not FDA-approved. (4) NO PHARMACY OR CLINICIAN NAMED IN ADVANCE. The pharmacy is "named on the prescription label and the shipping paperwork", and no clinician is named anywhere; FormBlends says it has no named medical reviewers yet. Prescriptions come from Receipt MD LLC. (5) WHERE IT SHIPS. FormBlends says it has dispensed to patients in 36 states and territories; for 16 others, including Alaska, Hawaii, Louisiana, New Mexico, Oklahoma and DC, it asks you to check at assessment. (6) LAB WORK IS NOT INCLUDED. (7) A WIDER CATALOGUE. FormBlends also sells peptides such as BPC-157 and sermorelin; this review covers its GLP-1 program only. WHAT WE CHECKED AND FOUND CLEAN: every page that quotes brand-drug trial results (STEP 1, SURMOUNT-1) also states that compounded semaglutide and tirzepatide are not FDA-approved and are not generic versions of Wegovy, Ozempic, Zepbound or Mounjaro. The FDA warning-letter database has no letter to FormBlends, Form Blends LLC or Receipt MD. The operating company is Form Blends LLC, 30 N Gould St, Ste R, Sheridan, WY, and the Terms and refund policy are dated 5 September 2026. What the research says

Dr. Daniel J. Drucker (Lunenfeld-Tanenbaum Research Institute, University of Toronto. Better Business Bureau

Pharmacy partner

FormBlends does not name the specific pharmacy that fills or compounds its medications (checked formblends.com/pharmacy-and-quality, /pricing, /terms, 2026-10-14). That is the norm for this category, not unusual for FormBlends specifically. Most providers we've checked don't name one either. "Medication is dispensed by state-licensed 503A compounding pharmacies"; "The dispensing pharmacy for any order is named on the prescription label and the shipping paperwork." No pharmacy is named before an order. Support gives the licence number and the lot's certificate of analysis on request.

HIPAA disclosure

Published. A real HIPAA-specific notice, checked 2026-10-14. A "HIPAA Notice of Privacy Practices" covering "FormBlends and the licensed pharmacy partners and healthcare providers who deliver services through the FormBlends platform", with the patient's PHI access rights.

How FormBlends works

  1. 1. Online intake. Complete a medical questionnaire and eligibility check (often BMI-based) covering your health history.
  2. 2. Clinician review. A licensed clinician reviews your information and, if appropriate, prescribes semaglutide or tirzepatide with a dose-titration plan.
  3. 3. Delivery & monitoring. Your medication ships discreetly to your door, with check-ins and dose adjustments along the way.

Frequently asked questions

How much does FormBlends cost?

Compounded semaglutide is $99 for the first month, then $190 a month, or $1,188 for a 12-month plan ($99 a month); 3 and 6 month plans are $399 and $714. Compounded tirzepatide is $149 for the first month, then $239 a month, or $2,148 for 12 months ($179 a month). Each plan is paid once, up front, and includes the medication, supplies, the provider review and shipping. Lab work is not included. Read 14 October 2026.

Is FormBlends legit?

FormBlends is Form Blends LLC, a Wyoming company. Prescriptions come from clinicians of Receipt MD LLC and medication from 503A compounding pharmacies, which are named on your prescription label rather than in advance. We found no FDA warning letter to either company. Its sign-up page shows a 4.8 Trustpilot rating, but Trustpilot has no page for formblends.com, and BBB has no profile for it (read 14 October 2026).

Can I cancel FormBlends?

Plans are one-time charges for their full length, so cancelling stops the next charge rather than refunding a plan already paid. Subscribe & Save renewals can be cancelled by email before the next billing date. Medication that has been dispensed cannot be returned or refunded; if the provider does not prescribe, the whole charge is refunded.
