Short answer

Pomegranate Health scores 7.2/10 (#26 of 53 clinics). $73/mo on the $219 3-month plan is the lowest tier Pomegranate Health lists, but the page doesn't spell out the commitment behind it. Treat it as a floor, not a quote for what you'd actually pay. There is also a flagged concern. See Is Pomegranate Health legit? below.

Verified live 2026-10-04 on app.joinpomegranate.com/pricing (linked as Pricing in the site navigation), clicking every pharmacy, dose tier, plan length and variant. Pomegranate prices each compounded medication separately for six named pharmacies, two unexplained dose tiers (Starter and Advanced) and up to four plan lengths. $73/mo is the lowest published rate: compounded semaglutide (with glycine or B12), Starter tier, Hyalo pharmacy, billed $219 for a 3-month plan; Hyalo offers no 1-month plan. The lowest month-to-month semaglutide rate is $99 (Hallandale, Starter), which does not ship to AL, AR, CA, HI, KS, MA, MI, MN or MS. The page opens on Empower at $129/mo. Compounded tirzepatide starts at $139/mo (Hallandale) or $299 for 3 months (Hyalo). The product pages print different headlines (semaglutide "Starts @ $119/mo", tirzepatide "Starts @ $159/mo"). The pricing page lets you pick a pharmacy, but the Subscription Terms say "Pomegranate Health cannot guarantee which pharmacy medication will be sourced from" and charge a $150 fee to specify one, so the cheapest pharmacy is not a price you can count on. Surcharges for Alaska, Hawaii and Puerto Rico are $50 to $75 depending on pharmacy. Medication fees are non-refundable, and multi-month plans are paid up front.

Questions to ask Pomegranate Health at intake

Each one is here because Pomegranate Health’s pages left it open when we read them.

  1. Do I need lab work before starting, and what does it cost? Its pages don't say whether labs are needed or what they cost.
  2. Is the medication included in this price, or billed separately? We didn't find a statement of what the price covers.
  3. Does my price change when my dose goes up? Its plan prices don't say whether they hold at higher doses.
  4. Can I use insurance or an HSA/FSA card for any part of this? We didn't find a statement from Pomegranate Health about insurance.
  5. How do I reach a clinician between visits, and how fast do they reply? We didn't find a published support or messaging commitment.

What we checked before publishing this page.

Independently verified on Trustpilot: 4.0/5 from 216 reviews (checked 2026-10-04). 143 of those (66%) are from the last 12 months. A rating resting mostly on old reviews reads differently than one built on recent ones. Profile listed as "Join Pomegranate" (claimed February 2024) and covers the whole brand, including weight loss, women's health, skin and longevity, not just GLP-1. Distribution 69% five-star, 20% one-star. Trustpilot notes "No recent history of asking for reviews" and "Replied to 15% of negative reviews". Two September 2026 reviews describe a $75 appointment/provider fee, which the site's "No additional fees or membership costs" does not mention. See reviews →

Watch out for

Pomegranate publishes more of its price structure than most clinics here, and less about everything around it. (1) BBB RATES THE COMPANY F. The BBB profile for Pomegranate Health in Jacksonville, FL (same phone number as the site, 813-419-1189) shows an F rating, not accredited, with 14 complaints closed in the last 3 years (bbb.org, read 2026-10-04). (2) THE PHARMACY DECIDES YOUR PRICE, AND YOU MAY NOT GET TO CHOOSE IT. The pricing page shows a pharmacy selector, and the same Starter semaglutide plan ranges from $73/mo (Hyalo, 3-month plan) to $149/mo (Red Rock, monthly). The Subscription Terms say Pomegranate "cannot guarantee which pharmacy medication will be sourced from" and charge a $150 fee if you want to specify one. Each pharmacy also excludes different states, so in California Empower, Hallandale and NOVA are unavailable, including the $99/mo monthly option. (3) "NO MEMBERSHIP" VERSUS THE TERMS. The homepage says "No Membership required" and the product pages say "No additional fees or membership costs", but the Subscription Terms state that "Membership and one- time consultation fees are nonrefundable" without saying what those fees are. Two Trustpilot reviewers (September 2026) describe a $75 appointment fee that was supposed to be credited against the medication. (4) THREE DIFFERENT STARTING PRICES. The semaglutide product page says "Starts @ $119/mo", the pricing page opens at $129/mo and its grid goes down to $99/mo monthly. Tirzepatide is "Starts @ $159/mo" on its product page and $139/mo on the grid. (5) STRICT AFTER-SALE TERMS. Medication fees are non-refundable, problems must be reported within 24 hours of delivery, there is a $25 fee for appointments cancelled within 24 hours, a $100 fee for chargebacks the company wins, and the terms threaten a $500 "inconvenience fee" in small claims court. (6) SMALLER INCONSISTENCIES. The compounded product pages say "Direct from FDA-licensed manufacturer" while the same page's FAQ says the drug comes from compounding pharmacies; the site claims both 100,000+ and 350,000+ patients; the Wegovy pill and Foundayo pages both claim "14-15% of your body weight on average in 1 year*" with no footnote for the asterisk. No clinician or medical group is named anywhere; the Terms say only "a Pomegranate Health affiliated physician practice". WHAT WE CHECKED AND FOUND CLEAN: no FDA warning letter to Pomegranate Health (exact-phrase search of fda.gov's warning-letter database, 2026-10-04). One named pharmacy, Empower, has its own FDA warning letters; see the separate note. Trustpilot: 4.0 from 216 reviews on the claimed joinpomegranate.com profile, 143 in the last 12 months. FDA warning letters against a named fulfillment pharmacy. Not against Pomegranate Health

Pomegranate Health names its partner pharmacies in its own Terms of Service. One of them holds 3 FDA warning letters, none of which has been closed out. This is a fact about the pharmacy that fills the prescription, not a finding against Pomegranate Health, and it does not change Pomegranate Health's score.

Empower Clinic Services, LLC dba Empower Pharmacy. MARCS-CMS 738238, issued 2026-09-18

One of six pharmacies Pomegranate names by brand ("Empower") on its own pricing page, where the patient can pick a pharmacy; Pomegranate publishes no address or facility for it, and its Subscription Terms say it cannot guarantee which pharmacy fills an order. Empower's 503A facility at 7601 N. Sam Houston Pkwy W., Houston is the one FDA inspected. Issued by Center for Drug Evaluation and Research (CDER). Subject: CGMP/Finished Pharmaceuticals/Adulterated/Unapproved New Drug. Posted 2026-09-22. No closeout letter posted.

Following an inspection of the Houston facility at 7601 N. Sam Houston Pkwy W., Ste. 100 from November 3 to November 14, 2025, FDA states that the pharmacy "compounded drug products that are essentially copies of commercially available drug products regularly and in inordinate amounts", naming TIRZEPATIDE/NIACINAMIDE and SEMAGLUTIDE/CYANOCOBALAMIN injections, so those products lost the section 503A exemptions and are described as unapproved new drugs and misbranded. FDA also states that sterile products were "prepared, packed, or held under insanitary conditions, whereby they may have become contaminated with filth or rendered injurious to health" (inadequate smoke studies in the ISO 5 area, media fills not run under the most challenging conditions) and lists five CGMP violations. FDA found some corrective actions adequate, could not evaluate others, and called the documentation of the January 2026 media fill deficient.

Empower Clinic Services, LLC dba Empower Pharmacy. MARCS-CMS 700964, issued 2025-04-02

Subject: Compounding Pharmacy/Adulterated Drug Products. Posted 2025-04-29. No closeout letter posted.

Following an inspection from September 24 to October 4, 2024, FDA states that drug products intended or expected to be sterile were "prepared, packed, or held under insanitary conditions, whereby they may have become contaminated with filth or rendered injurious to health," making them adulterated under FDCA 501(a)(2)(A). Three specific observations: routine environmental monitoring did not sample within the critical area where aseptic processing occurs; media fills were not performed under the most challenging or stressful conditions, so there is "a lack of assurance that your firm can aseptically produce drug products"; and the stopper sorting bowl, supply hopper and insertion station contacting stoppers used for injectable products were not sterilized. FDA found some corrective actions adequate but could not fully evaluate others for lack of supporting documentation.

Same pharmacy, earlier letter, citing a different Houston facility (5980 West Sam Houston Parkway North). Recorded so the count is complete. Issued by Office of Pharmaceutical Quality Operations, Division II. Subject: Compounding Pharmacy/Adulterated Drug Products. Posted 2021-10-26. No closeout letter posted.

Earlier letter to the same company on the same subject, at a different Houston facility. Recorded here because it establishes that the 2025 letter is the second on adulterated compounded drug products at this pharmacy, not an isolated finding.

What the research says

Dr. Daniel J. Drucker (Lunenfeld-Tanenbaum Research Institute, University of Toronto. Co-discoverer of GLP-1 biology) reviewed the expanding evidence base for GLP-1 medicines beyond weight loss, including cardiovascular and kidney-disease benefit, alongside open questions on long-term safety that still require more data.

Transparency is capped at 5.0. On its own blog article comparing semaglutide and tirzepatide, which ends with a pitch for its tirzepatide program (compounded only: it sells no brand-name tirzepatide), Pomegranate Health says “The SURMOUNT-1 trial tested tirzepatide in adults with obesity or overweight plus at least one weight-related condition. At the highest dose (15 mg weekly), participants lost an average of 22.5 percent of their body weight over 72 weeks.” and “The STEP 1 trial for semaglutide at 2.4 mg weekly showed average weight loss of 14.9 percent over 68 weeks.” The product is compounded, and the same page does not say it is not FDA-approved. FDA's March 2026 warning letters to telehealth companies cited claims implying sameness with FDA-approved drugs. We apply this cap to every clinic, partner or not.
